Patient Travel Subsidy Scheme (PTSS)
These materials have been developed as part of the PTSS reform project and aim to improve the way people access PTSS subsidies and information.
The five forms in the PTSS process for patients are:
- The Patient Registration (Form A) that gets filled out by the patient to register for PTSS, and only needs to be filled out once and if the patient’s details need updating,
- The Travel Referral (Form B) is filled out by the clinician and includes the patient’s referral as well as details on their travel requirements, and
- The Appointment Attendance (Form C) which is completed by the patient’s treating specialist as confirmation the patient attended the appointment.
- The Accommodation Attendance (Form D) is filled out by clinicians to confirm patients have attended their approved accommodation.
- The Repatriation Request (Form E) is to be completed by the administering Hospital and Health Service (HHS) to support the repatriation of a deceased patient.
